Psychiatry billing looks simple on the surface, a small set of codes used over and over, but the rules behind those codes are strict, and the specialty is audited heavily on them. Individual psychotherapy codes are chosen by documented minutes, not by the appointment slot. Medication management visits combine evaluation and management coding with psychotherapy add-ons that follow their own time rules. A note that is clinically complete but missing start and stop times can still downcode or deny.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for psychiatry practices, built around time based psychotherapy coding, medication management, and the documentation habits that survive payer review.
Psychiatry billing challenges
Individual psychotherapy is billed by documented psychotherapy time: 90832 for 16 to 37 minutes, 90834 for 38 to 52 minutes, and 90837 for 53 minutes or more. Under 16 minutes, no psychotherapy code applies. The code follows the minutes in the note, not the scheduled length, so a 45 minute session scheduled as an hour still bills as 90834, and a session only becomes 90837 once documented time reaches 53 minutes. Time spent on documentation, phone calls, or record review before or after the session does not count toward psychotherapy time for standalone codes. Missing or vague time documentation is one of the most common audit findings in the specialty, and it drives downcoding from 90837 to 90834 as well as outright denials.
Medication management visits add a second layer. When a psychiatrist or other prescribing clinician provides an evaluation and management service together with psychotherapy in the same visit, the psychotherapy is billed as an add-on: +90833 with 16 to 37 minutes of psychotherapy, +90836 with 38 to 52 minutes, or +90838 with 53 minutes or more. The add-on is never billed on its own, and evaluation and management time does not count toward psychotherapy time. The evaluation and management level itself is selected on medical decision making. Diagnostic evaluations use 90791 without medical services or 90792 when the evaluation includes medical services, and the two are not billed together for the same patient on the same date.
The wider code set needs the same discipline. Family psychotherapy uses 90846 without the patient present and 90847 with the patient present, group psychotherapy uses 90853, and crisis psychotherapy uses 90839 for the first 60 minutes with 90840 for each additional 30 minutes. Interactive complexity, billed with add-on 90785, applies only when communication factors substantially increase the work, and the note must describe those factors. Telehealth adds place of service and modifier requirements that vary by payer, and commercial payers do not always mirror Medicare rules, so each payer’s current policy must be confirmed rather than carried forward from a prior year.

Frequently asked questions
How is the psychotherapy code chosen?
By documented psychotherapy minutes: 16 to 37 minutes is 90832, 38 to 52 minutes is 90834, and 53 minutes or more is 90837. The appointment slot does not decide the code.
Can a medication management visit include psychotherapy?
Yes. The evaluation and management code is billed with a psychotherapy add-on, +90833, +90836, or +90838, matched to the psychotherapy minutes. Only psychotherapy minutes count toward the add-on.
What is the difference between 90791 and 90792?
Code 90791 is a psychiatric diagnostic evaluation without medical services, and 90792 is the same evaluation with medical services such as medication review and prescribing. Only clinicians with prescribing authority bill 90792.
Why do 90837 claims get downcoded?
Because the note does not clearly establish that the session reached 53 minutes. Documenting actual start and stop times, not just an estimated duration, is what defends the code.
Do telehealth psychiatry visits use the same codes?
Generally yes, with payer specific place of service and modifier requirements. Medicare and commercial rules differ, so we verify each payer’s current telehealth policy.
