Urology Billing Services
Urology billing covers office visits, endoscopic procedures, stone management, prostate surgery, urodynamics, and oncology, and the procedures often happen in combinations that trigger bundling edits. A diagnostic cystoscopy bundled into a therapeutic one, a stent billed alongside a stone removal it belongs to, or a prostate biopsy coded from an outdated superbill can each deny a claim that was clinically straightforward. The specialty also changes fast: code families are rebuilt from time to time, and a superbill that was correct last year can generate invalid code denials this year.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for urology practices, built around cystoscopy and prostate procedure rules, stone management bundling, and urodynamics documentation.
Urology billing challenges
Cystoscopy coding starts with the diagnostic versus therapeutic distinction. Diagnostic cystourethroscopy is billed with 52000, while therapeutic cystourethroscopy codes cover biopsy (52204), fulguration, resection, and stone removal (52204 to 52287). The diagnostic code is included in the therapeutic codes and must not be billed independently alongside them. Ureteroscopy follows the same logic: diagnostic ureteroscopy (52351) differs from ureteroscopy with stone treatment by lithotripsy (52353), and stent placement at the same session as stone removal is generally not separately billable unless it is unrelated. Stone management also includes extracorporeal shock wave lithotripsy (50590), and each stone procedure must be coded to the approach and technique actually used.
Prostate procedures demand current code sets. Transurethral resection of the prostate is billed with 52601 and laser vaporization with 52648, and prostate biopsy coding was rebuilt in the current CPT code set: the older single biopsy code was deleted and replaced with a family of codes that distinguish approach and imaging guidance, plus an add-on code. A superbill built on the older code will now generate invalid code denials, and the operative note must state the approach and guidance method plainly, because those facts now determine code selection. Documentation for prostate biopsy and cystoscopy with biopsy should also establish medical necessity through pre-procedure findings, such as urinalysis results or prior pathology, since payers commonly require documented indication and, for non-cancer indications, documented failure of conservative management.
Urodynamics and the remaining procedure families add documentation driven rules. Urodynamic studies use codes 51726 to 51797, and each component must be documented to support the code billed, while post-void residual measurement by ultrasound uses 51798 and catheter insertion and change codes (51701 to 51710) are often bundled into major surgeries. Laterality modifiers RT and LT apply to paired organ procedures such as vasectomy (55250) and orchiectomy, and modifier 25 supports a significant, separately identifiable evaluation and management service on the same day as a procedure, while modifier 59 marks a genuinely distinct service. Prior authorization is also common for advanced prostate treatments and imaging, and the authorization must match the procedure and diagnosis before scheduling.
What MedPlus RCM handles for urology practices
- End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing, the full cycle.
- Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
- Provider credentialing and enrollment. End-to-end enrollment with commercial payers and Medicare, plus re-credentialing maintenance.
- Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
- A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
- Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.
We serve urology practices nationwide, including general urology, urologic oncology, endourology, female urology, and pediatric urology practices, across prostate disease, stone disease, bladder conditions, incontinence, and male reproductive health.

Frequently asked questions
Can diagnostic and therapeutic cystoscopy be billed together?
No. Diagnostic cystoscopy (52000) is included in the therapeutic cystourethroscopy codes and must not be billed independently alongside them.
Why are prostate biopsy claims denying on code validity?
Because the prostate biopsy code family was rebuilt in the current CPT code set. The older single code was deleted and replaced with codes that distinguish approach and imaging guidance, so superbills and templates built on the old code must be updated.
Is a stent billed separately with stone removal?
Generally no. Stent placement at the same session as stone removal is not separately billable unless it is unrelated to the stone procedure.
What documentation supports a cystoscopy with biopsy?
Pre-procedure findings such as urinalysis results, prior cystoscopy findings, or imaging, plus the indication, biopsy sites, and tissue characteristics in the operative note, with pathology orders showing diagnostic intent.
Do urology procedures need prior authorization?
Many advanced prostate treatments and imaging studies do. Authorization should match the procedure and diagnosis before scheduling, and non-cancer indications commonly require documented failure of conservative management.
