Wound Care Billing Services
Wound care billing is decided by measurement, not by diagnosis alone. The code for a debridement depends on the depth of tissue removed and the surface area treated, and a note that describes the wound vaguely will default to the lowest supportable code or deny outright. Add skin substitute applications with their own medical necessity rules, multiple wounds in one visit, and same-day evaluation rules, and wound care becomes a specialty where documentation quality is the revenue cycle.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for wound care practices, built around the measurement and documentation rules that actually decide whether wound care claims pay.
Wound care billing challenges
Debridement coding splits into two families, and choosing between them is the first decision on every claim. Surgical or excisional debridement uses codes 11042 through 11047, selected by tissue depth, from subcutaneous tissue through muscle to bone, and by surface area. Selective debridement uses codes 97597 and 97598, for removal of devitalized tissue from open wounds, also by surface area. The deepest tissue layer debrided determines which code applies, and the note must state that depth directly, not imply it. Without a stated depth, payers default to the lowest supportable code.
Surface area rules differ between the families, which is where multi-wound visits go wrong. For surgical debridement, wounds debrided to the same depth are combined to determine the code, while wounds at different depths are coded separately by depth. For selective debridement, the total surface area of all wounds in the session is combined regardless of depth. Coding is based on wound size and depth, not on time spent, and the standard measurement is the greatest length by the greatest width. Skin substitute graft applications, coded 15271 through 15278 by wound size and location, add a further layer, generally requiring documentation of medical necessity and prior therapy failure, with payer rules varying.
Same-day evaluation rules complete the picture. An evaluation code is not usually billed with a debridement procedure. When a genuinely separate and reasonable evaluation is provided on the same day, it is payable only when the documentation clearly establishes it as separately identifiable and distinct from the debridement service, with modifier 25 on the evaluation code.
What MedPlus RCM handles for wound care practices
- End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing, the full cycle.
- Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
- Provider credentialing and enrollment. End-to-end enrollment with commercial payers and Medicare, plus re-credentialing maintenance.
- Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
- A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
- Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.
We serve wound care practices of every size, including dedicated wound clinics, podiatry-linked wound services, hospital-based wound centers, long-term care wound programs, and home-based wound care providers.

Frequently asked questions
What decides the debridement code: time or size?
Size and depth, not time. The code depends on the tissue layer reached and the surface area treated, measured as greatest length by greatest width.
What is the difference between 11042 and 97597?
11042 is surgical debridement to subcutaneous tissue, while 97597 is selective debridement of an open wound. The technique and the depth documented in the note decide which family applies.
How do we code multiple wounds in one visit?
For surgical debridement, combine the area of wounds debrided to the same depth, and code different depths separately. For selective debridement, combine the total area of all wounds in the session.
Can we bill an office visit on the same day as debridement?
Only when the evaluation is significant, separately identifiable, and distinct from the debridement service, with modifier 25 on the evaluation code and documentation to support it.
How long does it take to switch our billing to MedPlus RCM?
Onboarding takes between 2 and 4 weeks depending on practice size, including data migration and payer clearinghouse setup.
What does MedPlus RCM charge?
A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
