Sports Medicine Billing Services

Sports medicine billing turns on two questions: which joint was injected, and was ultrasound guidance really performed and documented. The injection code family is organized as a grid of joint size against guidance, and picking the wrong cell, or billing guidance that was not properly documented, is the most common audit finding in the specialty. Add tendon and trigger point injections, diagnostic ultrasound, and biologic therapies with uneven coverage, and a busy injection clinic can generate a surprising number of ways to lose revenue.

MedPlus RCM provides medical billing, coding, credentialing, and denial management for sports medicine practices, built around the specialty’s actual code families and payer requirements.

Sports Medicine billing challenges

Joint injection and aspiration codes are sized by joint and split by guidance. Small joints and bursae, such as fingers and toes, use 20600 without guidance and 20604 with ultrasound guidance. Intermediate joints and bursae, such as the wrist, elbow, ankle, and acromioclavicular joint, use 20605 without guidance and 20606 with guidance. Major joints and bursae, such as the shoulder, hip, knee, and subacromial bursa, use 20610 without guidance and 20611 with guidance. The guided codes require all three elements: ultrasound used for needle guidance, a permanently saved image, and a separate guidance report. If any element is missing, the defensible code is the unguided version, and billing a guided code without the saved image is a classic audit finding.

Ultrasound guidance for other injections follows a different pattern. Code 76942, ultrasonic guidance for needle placement, is reported alongside injection codes that do not already include guidance, such as tendon sheath injections (20550 and 20551) and trigger point injections (20552 for 1 or 2 muscles and 20553 for 3 or more muscles). But 76942 must not be added to the guided arthrocentesis codes, because guidance is already built into those codes, and billing both is a bundling violation. Diagnostic musculoskeletal ultrasound is separate again: 76881 for a complete diagnostic exam and 76882 for a limited one. If ultrasound is used only to guide the needle, the guidance code applies, not a diagnostic code. A diagnostic ultrasound and a guided injection can both be billed on the same day only when the documentation clearly separates the diagnostic portion from the guidance portion.

Biologic therapies add coverage uncertainty. Platelet-rich plasma injection is reported with Category III code 0232T, which includes image guidance, harvesting, and preparation when performed, but coverage varies widely by payer and many plans consider it experimental or investigational, so benefits must be verified before treatment. Bilateral injections follow payer-specific bilateral rules, and different joints treated on the same day need separate lines with distinct documentation for each joint.

What MedPlus RCM handles for Sports Medicine practices

  • End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing, the full cycle.
  • Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
  • Provider credentialing and enrollment. End-to-end enrollment with commercial payers and Medicare, plus re-credentialing maintenance.
  • Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
  • A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
  • Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.

Frequently asked questions

What makes an ultrasound-guided injection billable as guided?

Three elements must all be present: ultrasound used for needle guidance, a permanently saved image, and a separate guidance report. Missing any element means the unguided code should be billed instead.

Can ultrasound guidance be billed separately with a guided joint injection code?

No. The guided arthrocentesis codes already include guidance, so adding 76942 to them is a bundling violation. Code 76942 is for injections whose codes do not already include guidance.

How are bilateral injections billed?

Bilateral rules vary by payer, and the claim must follow the specific payer’s bilateral policy. Different joints on the same day need separate lines with distinct documentation for each joint.

Is platelet-rich plasma therapy covered?

Coverage varies widely. Many payers consider platelet-rich plasma experimental or investigational, so benefits and authorization should be verified before treatment.

What does MedPlus RCM charge?

A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
