Telehealth & Remote Patient Monitoring Billing Services

Telehealth billing fails on details that have nothing to do with the clinical visit. The wrong modifier, the wrong place of service, a missing consent note, or a monitoring threshold missed by a day will deny a claim for a visit that was delivered perfectly. Remote patient monitoring adds a second layer of thresholds: days of data, minutes of management time, and one billing practitioner per patient per period. Practices that treat virtual care as “the same visit, just on video” lose revenue on exactly those details.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for telehealth and remote patient monitoring programs, built around the modifier, place-of-service, and threshold rules that actually decide whether virtual claims pay.

Telehealth & remote patient monitoring billing challenges

Video visits use the standard evaluation codes, 99202 through 99215, but the claim must also carry the right telehealth signals. Modifier 95 marks synchronous audio-video telemedicine, modifier 93 marks audio-only services, and some payers still require the GT modifier, so mixing them up or skipping them triggers rejections. Place of service matters just as much: place of service 10 is used when the patient is at home, and place of service 02 when the patient is at another facility, and the wrong choice can change the payment or deny the claim. Documentation must record patient consent, the technology platform used, the patient and provider locations, the visit duration, and the medical decision making. Audio-only visits, brief virtual check-ins, and online digital visits each have their own code families and payer acceptance rules, so the visit type must be matched to the code before submission, not after.
Remote patient monitoring is threshold billing. Device setup is reported with code 99453, device supply with 99454 when at least 16 days of data are transmitted in a 30-day period, and management time with 99457 for the first 20 minutes of interactive communication in the month, plus 99458 for each additional 20 minutes. Only one practitioner may bill a patient’s remote monitoring in a given period, patient consent is required before billing, and monitoring time must be documented separately from other care management time billed in the same month. Remote therapeutic monitoring uses a separate code family for therapy-related devices, and its management codes cannot be billed alongside remote patient monitoring management codes for the same patient in the same month.

Frequently asked questions

What is the difference between place of service 02 and 10?
Place of service 10 is used when the patient is at home, and place of service 02 when the patient is at another facility. Using the wrong one can change payment or deny the claim.
Which modifier goes on a video visit?
Modifier 95 for synchronous audio-video visits is the most common, modifier 93 marks audio-only services, and some payers require GT. We apply the modifier each payer requires.
What thresholds must remote patient monitoring meet?
Device supply code 99454 requires at least 16 days of data in a 30-day period, and management code 99457 requires the first 20 minutes of interactive communication in the month. Only one practitioner bills a patient’s monitoring in a given period.
Can remote patient monitoring and chronic care management be billed together?
Yes, when the time is documented separately for each program. Monitoring time and care management time must not overlap on the same minutes.
How long does it take to switch our billing to MedPlus RCM?
Onboarding takes between 2 and 4 weeks depending on practice size, including data migration and payer clearinghouse setup.
What does MedPlus RCM charge?
A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
