Medical Billing Services in Hawaii
Hawaii’s coverage landscape is shaped by its islands. Medicaid runs through a single managed care program with five competing health plans, and plan availability varies by island. A practice on Oahu and a practice on the Big Island can face different plan options for the same patient population, and patients sometimes travel between islands for specialty care. Verifying the plan and its network before the visit is what keeps those claims paid.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for Hawaii practices of every specialty, built around the state’s actual payers and programs.
Hawaii’s payer landscape
Hawaii’s Medicaid program is Med-QUEST, delivered through QUEST Integration, a unified managed care program covering most Medicaid eligible populations. Members choose one of five health plans at enrollment: AlohaCare, HMSA (Hawaii Medical Service Association), Kaiser Permanente Hawaii, ‘Ohana Health Plan, and UnitedHealthcare Community Plan. The chosen plan coordinates the member’s covered care. Plan availability is not identical across islands. Kaiser Permanente’s QUEST plan, for example, is available on Oahu and Maui only, so a patient who moves islands or seeks care off island can fall outside their plan’s network if the change is not caught first.
Commercial coverage is led by HMSA, the Blue Cross Blue Shield licensee for the state and its largest commercial insurer, alongside Kaiser Permanente, UnitedHealthcare, HMAA, and UHA. Hawaii’s long standing employer coverage mandate means most full time workers carry employer sponsored insurance, which adds a large group coverage book alongside the individual and Medicaid markets.
What MedPlus RCM handles for Hawaii practices
- End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing. That is the full cycle.
- Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
- Provider credentialing and enrollment. End-to-end enrollment with QUEST Integration plans, commercial payers, and Medicare, plus re-credentialing maintenance.
- Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
- A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
- Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.

Frequently asked questions
Why is Hawaii Medicaid billing different?
QUEST Integration has five health plans, and availability varies by island. A member’s plan, and whether that plan’s network covers care on another island, has to be verified before the visit. Billing the wrong plan or assuming statewide network coverage produces denials that have nothing to do with the care itself.
Do you work with QUEST Integration plans?
Yes. We bill AlohaCare, HMSA, Kaiser Permanente Hawaii, ‘Ohana Health Plan, and UnitedHealthcare Community Plan, as well as commercial and Medicare plans.
How long does it take to switch our billing to MedPlus RCM?
Onboarding takes between 2 and 4 weeks depending on practice size, including data migration and payer clearinghouse setup.
What does MedPlus RCM charge?
A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
