Internal Medicine Billing Services

Internal medicine billing is dominated by complexity. Adult patients with multiple chronic conditions, medication lists that change every visit, and care that spans the office, the hospital, and the phone call in between all land on the same claim set. The revenue risk is not a single tricky procedure, it is the steady undercoding of complex visits and the care management work that happens outside the exam room.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for internal medicine practices, built around high-complexity adult care.

Internal medicine billing challenges

Evaluation and management level selection is the first challenge. Since the 2021 E/M revisions, office visit levels rest on medical decision-making or total time, and internists routinely manage multiple chronic conditions with significant risk. Notes that list problems without showing what was managed at the encounter understate the visit, and undercoding becomes a permanent revenue leak.
Care management programs are the second challenge. Chronic care management covers monthly non-face-to-face coordination for patients with two or more chronic conditions, and transitional care management covers the period after a hospital discharge, with an interactive contact within two business days and a face-to-face visit within a set window. Both programs have strict timing, consent, and documentation rules, and both are frequently performed but never billed.
A third challenge is the hospital and office mix. Internists who round in the hospital and see patients in the office must keep inpatient, observation, and outpatient coding distinct, and must not double-count work that belongs to a single encounter. Supervision rules also matter when nurse practitioners, physician assistants, or clinical staff deliver parts of the care, since incident-to billing requires the supervising physician’s ongoing involvement and presence rules that vary by payer.

What MedPlus RCM handles for internal medicine practices

  • End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing, the full cycle.
  • Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
  • Provider credentialing and enrollment. End-to-end enrollment with commercial payers, Medicare, and Medicaid, plus re-credentialing maintenance.
  • Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
  • A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
  • Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.
We serve internal medicine practices of every size, from solo clinics to multi-provider groups, across all 50 states.

Frequently asked questions

Why do internal medicine visits so often get undercoded?
Because the note lists chronic problems without documenting what was managed at that encounter. E/M level follows medical decision-making or time, so the note must show the conditions addressed, the data reviewed, and the risk of the management decisions made.
What is transitional care management, and why is it missed?
Transitional care management covers care after a hospital discharge: an interactive contact within two business days of discharge and a face-to-face visit within 7 or 14 days, depending on complexity. It is missed when the contact or the visit falls outside the window, or when no one tracks the discharge at all.
Can chronic care management and an office visit be billed together?
Yes, with care. Chronic care management covers non-face-to-face coordination time in the calendar month, and the initiating visit is billed separately. Time counted toward chronic care management must not double-count time already used to select an E/M level.
Do you bill for hospital rounding as well as office visits?
Yes. We code inpatient, observation, and outpatient encounters distinctly, and we keep hospital and office work from overlapping on the same claim.
How long does it take to switch our billing to MedPlus RCM?
Onboarding takes between 2 and 4 weeks depending on practice size, including data migration and payer clearinghouse setup.
What does MedPlus RCM charge?
A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
