Otolaryngology Billing Services

Otolaryngology, or ENT, covers three anatomical territories, nose and sinuses, ears and hearing, and throat and voice, plus allergy services, and each territory has its own code families, global periods, and bundling rules. A sinus surgery claim can fail on the choice between diagnostic and surgical endoscopy codes. An allergy claim can fail on unit caps. A hearing claim can fail on laterality. The specialty rewards billing teams that know which territory each claim belongs to.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for otolaryngology practices, built around sinus surgery coding, audiology and vestibular testing, allergy services, and the global period rules that tie them together.

Otolaryngology billing challenges

Sinus surgery is the most complex area. Nasal and sinus endoscopy and functional endoscopic sinus surgery (FESS) use codes 31231 to 31288, including diagnostic endoscopy, ethmoidectomy, maxillary antrostomy, sphenoidotomy, and frontal sinus exploration, with balloon sinuplasty coded separately at 31295 to 31298. The critical rule is that FESS codes apply only when the procedure was actually performed and documented through the endoscope; procedures done through open approaches use different code families. Any diagnostic endoscopy performed during the same session as a surgical endoscopy is included in the surgery and is not billed separately. Most endoscopic sinus surgery codes carry a 0-day global period, meaning only services on the day of the procedure are associated with the code, while a small set of codes carries a 10-day period, so postoperative debridement and related visits must be handled according to the specific code’s period. Septoplasty (30520) and turbinate procedures (30130 and 30140) carry their own global periods and medical necessity requirements, and a septoplasty billed without a supporting diagnosis for obstruction will deny for medical necessity.
The ear and throat side brings its own families. Tympanostomy tube placement uses 69433 and 69436, laryngoscopy uses 31575 to 31579, and tonsillectomy and adenoidectomy use 42820 to 42836. Audiometry uses 92552 to 92557 and vestibular testing uses 92540 to 92548, with each test requiring the documented components that justify it. Allergy services use testing codes 95004 to 95028 and immunotherapy codes 95115, 95117, and 95165, and these are subject to unit caps and coverage policy changes that vary by Medicare contractor, so the testing volume and the immunotherapy preparation must both match the payer’s current policy.
Modifiers tie the territories together. Modifier 50 marks a truly bilateral procedure on a paired organ, while LT and RT mark side specific services, and confusing the two is a common denial cause. Modifier 25 supports a same day evaluation and management service with a minor procedure, modifier 59 and the XS modifier mark a distinct site or session, and modifiers 78 and 79 handle a return to the operating room or an unrelated procedure during a global period. For example, a sinus debridement after septoplasty may need modifier 79 to override the septoplasty global period, while no such modifier is needed after a turbinate procedure that shares the 0-day global period of most sinus surgery codes.

What MedPlus RCM handles for otolaryngology practices

  • End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing, the full cycle.
  • Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
  • Provider credentialing and enrollment. End-to-end enrollment with commercial payers and Medicare, plus re-credentialing maintenance.
  • Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
  • A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
  • Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.
We serve otolaryngology practices nationwide, including general ENT, rhinology, otology, laryngology, and pediatric ENT practices, across sinus disease, hearing loss, voice disorders, sleep apnea surgery, and allergy care.

Frequently asked questions

When do FESS codes apply?
Only when the sinus procedure was performed and documented through the endoscope. Open approaches such as Caldwell-Luc antrotomy use different code families, and coding them as endoscopic procedures will not survive review.
Can diagnostic and surgical endoscopy be billed together?
No. A diagnostic endoscopy performed during the same session as a surgical endoscopy is included in the surgery and is not reported separately.
What global period applies to sinus surgery?
Most endoscopic sinus surgery codes carry a 0-day global period, so only same day services are associated with the code. A small set of codes carries a 10-day period, so the specific code’s period must be checked before billing postoperative care.
What is the difference between modifier 50 and LT/RT?
Modifier 50 marks a truly bilateral procedure on a paired organ, while LT and RT mark side specific services. Using one where the other applies is a common denial cause.
Why do allergy claims deny on units?
Allergy testing and immunotherapy are subject to unit caps and contractor coverage policies that change over time. Testing volume and immunotherapy preparation must match the payer’s current policy.
