Endocrinology Billing Services

Endocrinology billing spans three different payment worlds at once: office visits, diabetes technology, and procedures, and each one has its own code families, frequency limits, and benefit rules. A practice that bills its diabetes devices like its office visits, or its thyroid procedures like its lab work, will see denials that have nothing to do with the care delivered and everything to do with which benefit the claim was routed through.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for endocrinology practices, built around diabetes technology, hormone management, and thyroid procedure coding.

Endocrinology billing challenges

Continuous glucose monitoring is the highest-volume trap. Three professional codes divide the work: 95249 for startup of a patient-owned device, 95250 for startup of a practice-provided professional device, and 95251 for interpretation of at least 72 hours of CGM data. Interpretation is generally billable no more than once per 30 days, and an evaluation and management service on the same date as CGM services needs modifier 25 and documentation showing it was significant and separately identifiable. Device and supply codes for receivers, transmitters, and sensors follow durable medical equipment rules, and the split between the DME benefit and the pharmacy benefit is a frequent denial source, since routing the claim to the wrong benefit reads as a coverage failure.
Diabetes education and nutrition services add their own rules. Diabetes self-management training and medical nutrition therapy each carry frequency limits and same-day conflict rules, and the two programs must not be billed in ways that overlap. Thyroid procedures require a different precision: fine needle aspiration codes and core biopsy codes are distinct, imaging guidance must not be unbundled from the aspiration where the payer bundles it, and modifier 25 applies when an evaluation and management service is reported on the same day as the aspiration.
Medication and lab management complete the picture. GLP-1 and insulin pump claims frequently require prior authorization, and pump and CGM coverage depends on documented criteria such as insulin treatment or problematic hypoglycemia. Hormone and diabetes lab work must be linked to the documented diagnosis, and chronic care management for patients with multiple endocrine and metabolic conditions follows the same consent, care plan, and monthly time rules as in primary care.

What MedPlus RCM handles for endocrinology practices

  • End-to-end medical billing. Charge entry, claim scrubbing, submission, payment posting, and patient billing, the full cycle.
  • Medical coding. Certified coders assign current ICD-10, CPT, and HCPCS codes matched to your documentation and specialty.
  • Provider credentialing and enrollment. End-to-end enrollment with commercial payers, Medicare, and Medicaid, plus re-credentialing maintenance.
  • Denial management and recovery. Every denial investigated to root cause, corrected, and appealed with documentation.
  • A/R recovery. Aged and outstanding balances audited and pursued, including balances already written off where the appeal window is still open.
  • Financial reporting. Clean claim rates, collections, denial patterns, and A/R aging visible by payer and provider.

We serve endocrinology practices of every size, from solo physicians to multi-provider groups, across all 50 states.

Frequently asked questions

What are the three CGM professional codes?
95249 covers startup of a patient-owned device, 95250 covers startup of a practice-provided professional device, and 95251 covers interpretation of at least 72 hours of CGM data. Interpretation is generally billable no more than once per 30 days.
Why do CGM supply claims deny?
Often because the claim was routed to the wrong benefit. CGM devices and supplies can fall under the durable medical equipment benefit or the pharmacy benefit depending on the payer and product, and routing to the wrong one reads as a coverage denial.
Can an office visit be billed on the same day as CGM services?
Yes, when the visit is significant and separately identifiable from the CGM service. Modifier 25 goes on the evaluation and management code, and the note must support both services.
Do insulin pumps and GLP-1 medications need prior authorization?
Frequently yes. Pump and CGM coverage depends on documented criteria such as insulin treatment or problematic hypoglycemia, and we verify those criteria before the claim.
How long does it take to switch our billing to MedPlus RCM?
Onboarding takes between 2 and 4 weeks depending on practice size, including data migration and payer clearinghouse setup.
What does MedPlus RCM charge?
A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
