Ambulatory Surgical Centers Billing Services
Ambulatory surgical center billing is facility billing, and it follows a different rulebook from physician billing. The center bills a facility fee for the operating room, nursing, supplies, and implants, while the surgeon and other providers bill their professional services separately. That split is the foundation of ASC revenue, and every rule that follows, from the covered procedures list to packaging to discontinued-procedure modifiers, exists because the facility claim and the professional claim are judged independently.
MedPlus RCM provides medical billing, coding, credentialing, and denial management for ambulatory surgical centers, built around the facility rules that actually decide whether ASC claims pay.
Ambulatory surgical centers billing challenges
The covered procedures list is the first gate. A procedure pays as an ASC service only when it sits on the current CMS ASC covered-procedures list with the correct payment indicator, and that list changes every year. A case that paid last year can shift indicator or drop off the list, and a center still billing it the old way collects nothing. Every scheduled procedure should be pre-checked against the current-year list before the date of service, not after the claim denies.
Packaging and device rules are the second gate. Packaged supplies and drugs are bundled into the facility payment and cannot be billed separately, while device-intensive cases need the implant invoice and the correct device coding to recover the device portion. Billing a packaged item as separately payable is a standard denial, and filing a device case without the invoice leaves the device value unrecovered. Multiple procedures in one session are reduced under the multiple-procedure rule, with the highest-weighted procedure paid in full and additional payable procedures reduced, so expected payment must be modeled per session, not per procedure.
Modifiers and authorization complete the picture. Discontinued procedures use modifier 73 when discontinued before anesthesia and modifier 74 when discontinued after anesthesia is induced, and both are audit-sensitive, requiring documentation of the reason. Laterality and distinct-service modifiers apply under standard editing rules. Commercial authorization must be verified per procedure and matched to the correct code and site of service before the case, since an authorization on the wrong code or site is one of the most common ASC denial causes.

Frequently asked questions
How is ASC facility billing different from surgeon billing?
The center bills a facility fee for the room, staff, supplies, and implants, while the surgeon bills a separate professional fee. The two claims are judged independently under different payment rules.
Why did a procedure that paid last year deny this year?
The ASC covered-procedures list changes annually. A procedure can shift payment indicator or drop off the list, so every scheduled case should be checked against the current-year list.
Can we bill supplies and drugs separately from the facility fee?
Packaged supplies and drugs are bundled into the facility payment and cannot be billed separately. Device-intensive cases are handled through device coding and the implant invoice.
What modifiers apply when a case is discontinued?
Modifier 73 when the procedure is discontinued before anesthesia, and modifier 74 when discontinued after anesthesia is induced. Both require documentation of the reason.
How long does it take to switch our billing to MedPlus RCM?
Onboarding takes between 2 and 4 weeks depending on practice size, including data migration and payer clearinghouse setup.
What does MedPlus RCM charge?
A performance-based percentage of successfully collected revenue. If you don’t get paid, we don’t get paid.
